Here’s a bit about what you can expect when you set up an appointment to work with us. When you call to make an appointment, we will begin by working with you to determine your insurance benefits and authorization needs.

When you come in to see us, you’ll be asked to complete any forms required, give us your insurance card and photo identification (for your protection), and asked if you have any questions. You may wish to complete the required paperwork prior to your first visit.

On that first appointment you will undergo a detailed and comprehensive evaluation. During the evaluation, we will be taking a health history, performing appropriate tests and measurements and developing a treatment plan which includes your goals. The evaluation and treatments run one hour per visit.

Insurance

We accept all insurance plans and we are preferred providers for: Workers Compensation, Medicare, Blue Shield, Health Net, and The Marin IPA. If you are not covered by any of these plans, we offer flexible interest free payment plans, or we’ll gladly accept MasterCard, Visa or American Express.

Below are the forms that can be downloaded and brought to your first visit. Our office staff will assist you in determining which forms should be completed. If you have any difficulty completing the forms, we will be happy to assist you either on the phone prior to your appointment or at the time of your first appointment. We recommend, in order to safeguard your personal information, that you do not email these forms to us.
