As of Friday, March 20, 2026, I retired and closed my surgical practice at the Wrist and Hand Center located at 4728 N. Habana Avenue, Suite 204, Tampa, FL 33614.

Your medical records are confidential and will be personally secured, stored in digital format, and retained for a minimum of five (5) years in accordance with Florida law.

For all records that have not been picked up prior to March 20, 2026 there will be a nominal fee, permitted by Florida law, to cover the cost of copying and mailing records. All requests for X-rays copied on CD will be charged a fee (plus postage if mailed) to cover the material costs and transference.
